A commercial bass fisherman i have been out with a few times used to hook live sandeels by putting a 3/0 through the mouth and out the gill (with out piercing the eel in any way), then putting the hook through the top of its spine half way down its back. You were almost guaranteed a fish on every bite.

with the pout and if there is no need to cast try just hooking under the dorsal fin, keeps the action in the livey going where as a hook through the nose slowes it down alot

The way monkeynuts descrbed is very effective, its the same way as they rig live tuna for big game fishing.

Do fish take their prey head first? If so does this mean that its important to have the hook facing the right way?

Yes, prey fish are swallowed head first. Better to hook through nostril/lip to keep livebait kicking and attract predators. Also, more humane. I've let many live-bait go after use none the worse, and probably very relieved fish!! ;D
